Maryland Immunization Certificate

We require a signed copy of your child’s immunization records. Feel free to use this form or another doctor-provided form. As long as your doctor has signed it, we can accept it.

Health Inventory OCC 1215 / Lead Test Form DHMH 4620

Part I of the Health Inventory must be completed by parents and Part II must be completed by the child’s pediatrician before your child may start care in Maryland. All children in Baltimore are required to get tested for lead exposure at 12 months (1 year) and 24 months (2 years). The Office of Child Care requires this form to be completed by your child’s doctor.

Medication Administration Form

This form must be completed before Friday’s Child can administer any over-the-counter or prescription medication. This form requires a signature from both the pediatrician and a parent. Topical OTC ointments are excluded.

Allergy Action Plan

This form must be completed, along with the Medication Form if your child has any allergies. We are not able to administer allergy medication (Benadryl/Epi-Pens) until these forms are completed. This form requires a signature from both the pediatrician and a parent.

Asthma Action Plan

This form must be completed, along with the Medication Form, if your child has asthma. We are not able to administer inhalers or asthma medication until these forms are completed. This form requires a signature from both the pediatrician and a parent.

MD Lead Test Form DHMH 4620
